The Remote Care Health Coach position is part-time, remote/work from home role. 

Key Responsibilities: 

  • Manage large caseload volumes of patients/members using Brook’s proprietary web-based tools. 
  • Be knowledgeable about each of the Brook programs, the Brook app and devices used to support patient/member’s health. 
  • Understanding and experience with chronic conditions as they relate to making sustainable lifestyle changes. 
  • Utilize real-time data from patients/members to provide proactive feedback in the form of regularly scheduled outreach either in app or via phone calls. 
  • Ensure all necessary documentation is completed accurately and promptly. 
  • Ensure patients/members achieve the programmatic goals in the most effective and sustainable manner. 

Qualifications: 

  • Minimum of 2 years of experience within a health coaching or similar role. 
  • Must hold an active National Board Certified Health and Wellness Coaching credential.
  • Experience in a client facing or telephonic role providing health and wellness coaching services.  
  • Proficient in utilizing remote technology and managing caseloads independently. 
  • Be available to work evenings and weekends in Eastern Standard Time. 

Preferred:

  • Experience with GLP-1 medications. 
  • Certification from the CDC to deliver the Diabetes Prevention Program. 

Once onboard, this person will be cross trained to work in many areas of our digital health platform supporting users in app, in our Remote Care Program and our other programs. 

This role is not eligible for visa sponsorship or relocation. This is a remote position and salary is commensurate with experience.
